The Muckleshoot Enrollment office carries out many different tasks required of us on a daily basis. The most important task required is to assist the Muckleshoot Enrollment Committee and Muckleshoot Tribal Council in maintaining the integrity of the Membership rolls. The Enrollment Department staff is required to uphold confidentiality at all times. We assist customers internally and externally in a professional and friendly manner.
Scope of Services. A few of our daily tasks include the following:
- Accurately issuing Tribal ID's to enrolled tribal members
- Updating address information as necessary
- Verifying enrollment
- Issuing Tax Exempt forms accurately to enrolled tribal members
- Issuing Enrollment applications
- Assisting applicants by informing them of what is necessary to accurately complete their files
- Answering questions in person and over the telephone as needed
- Work continuously to keep communication open with clients, other tribes, and BIA offices
Other important tasks that may not be completed daily include, but are not limited to:
- Tribal ID cards
As of February 2008, the Enrollment Office will issue tribal members 1 free ID card that will not expire for 4 years. The pricing for Tribal ID cards are as follows:
- 1st card, FREE
- 2nd card, $15.00
- 3rd card and any card after that, $20.00
- Tax Exempt forms
The Enrollment Office issues the following tax exempt forms:
- Vehicle/Vessel Excise Tax Exemption Affidavit for Enrolled Tribal Members Living in Indian Country
- Retail Sales Tax Exemption Certificate for Vehicles Sold to Enrolled Tribal Members by Auto Dealers
- Use Tax Exemption Certificate for Vehicles to Enrolled Tribal Members by Private Party
- Public Utility Tax Exemption for Services Provided to Indian Tribes and Indian Persons located on the Indian Reservation
- Issuing Enrollment Applications
When submitting an application, the following documents are required to accompany the application:
- Certified Birth Certificate
- Original Social Security Card
- The Enrollment Manager may require additional Documentation if necessary to verify or complete the information required in the application
- Additional information requested may include the establishment of paternity/maternity by DNA testing
- Enrollment Changes
- Address Change
- Enrollment Change form must be completed
- Name Change
- Enrollment Change form must be completed
- Original Social Security Card with new name must be submitted
- If name change occurs because of marriage, Marriage Certificate must be submitted
- If name change occurs because of a divorce, Divorce papers must be submitted
- Remember - all documents must be originals and must match, no acceptations
