Community Health focuses on assisting our tribal and community members to get medical services onsite, outside referrals, and follow-up after hospital admissions. Our program includes services provided by our community health representatives (CHR), diabetes nurse (RN), public health nurse (RN), medical social worker (MSW) and registered dietitian (RD).
